10 Steps to Post Acute Care Success in the New Age of Health Care

Strategies to develop an effective, efficient and successful post acute care model that will prevent readmission to acute-care settings post-discharge.

The delivery of care across the continuum is being transformed by new payment models. Post-acute care area is one of the most fragmented and siloed-off pieces of the health care spectrum, which impedes coordination of care and thus makes value-based care models that much more of a challenge.

Experts working in the value-based field say “Health plans are taking a more hands-on approach to integrating post-acute care — including long-term, skilled nursing facility (SNF) and rehabilitation care — into their strategies under pay-for-value arrangements. This new attention is predicated by the need to manage the total care continuum closer than under traditional fee-for-service (FFS) reimbursement, given the incentives to improve quality and especially to keep patients/members from being readmitted to acute-care settings post-discharge.”

Join us on this 1 hour webinar as Shawn Matheson of Leavitt Partners share insights on the 10 essential steps to providing successful post acute care.

You will receive insights and best practices to address:

  • Transitions of Care
  • Developing Clinical care Protocols

  • Provider Accountability
  • Compliance with MSSP (Medicare Shared Savings Program)
